HealthyRPM

HealthyRPM is DHCT's web-based CCM patient engagement application.  HealthyRPM is designed for Medicare reimbursement programs such as Chronic Care Management (CCM), Principal Care Management (PCM), and Behavioral Health. 

In addition to Medicare's telemedicine reimbursement programs, HealthyRPM's program management administration console allows our clients to build custom programs for potential clients such as commercial insurance companies seeking custom patient engagement programs, providers such as psychologists, chiropractors, and others seeking an online solution to track time-based patient engagements.
